Empowering residents and families on how to express their concerns and makes referrals to other appropriate community and advocacy agencies. Receiving and answering questions from individuals regarding financial, social, family, or legal issues. There are no age requirements or fees for this program, only that you or a loved one are a current resident or rehabilitation patient of a nursing home, adult home, assisted living residence, or licensed family type home.
Comprehensive training is required to become a Certified Ombudsman and is coordinated by the program. A weekly time commitment is also required to be a volunteer Ombudsman. The mission of the Long-Term Care Ombudsman Program is to seek resolution of problems and advocate for the rights of residents of long-term care facilities with the goal of ensuring their dignity, quality of life, and quality of care.
I ncreased isolation for our seniors can lead to unfortunate outcomes. All facilities nursing homes and assisted living facilities are strongly encouraged to provide access via other means for residents to stay connected - phone calls, Facetime, etc. We hope to purchase tablets for some of the 6-bed type facilities using the CARES funding we will receive to encourage face time, and other electronic means to connect residents with family.
The Ombudsman program will identify the facilities that would benefit from the gift of a tablet. The Ombudsman program investigates issues, speaking first to the resident if the resident has capacity - if not we will speak to the responsible party or conservator and proceed with the investigation of the issue.
Ombudsmen are advocates for people living in nursing and residential care homes. The program promotes the rights and well-being of residents in long term care facilities in Santa Clara County and provides residents with a means by which their complaints can be heard and resolved.
